The SoFlo Summer Shootout has chosen to use SPORT iD: the Player Safety App as the way for teams and players to sign tournament waivers, input player's medical history, and have rosters submitted as part of the pre-tournament check-in.

Simply ask your families to create an account and then player profile(s) including their child's valid US Lacrosse number. After that they simply Check In at the SoFlo Summer Shootout at which time they will be prompted to assign their child to the proper team and electronically sign the waiver.

We chose SPORT iD because it is an app focused on player safety. Using SPORT iD, your parents can sign tournament waivers and enter as much health and safety information as they like, which will be shared with us for the duration of the 2020 SoFlo Summer Shootout. In case of an emergency, team coaches and our medical staff can make better treatment decisions and easily contact a player's emergency contact.

Once the tournament is over, our access to your team members profile goes away.
